fa پایش شاخص‌های مهم کیفیت هوای داخل در بخش‌های مراقبت ویژه بیمارستان‌های منتخب شهر اصفهان و ارتباط آن‌ها با عوامل محیطی Monitoring Important Indoor Air Quality Parameters in the Intensive Care Units of Selected Hospitals in Isfahan City, Iran, and Their Relationship with Environmental Parameters مهندسی بهداشت محیط Environmental Health Engineering پژوهشي Research <div style="text-align: justify;">مقدمه: ارتقای کیفیت هوای داخلی بیمارستان‌ها در جهت حفاظت از سلامت بیماران، کارکنان و مراجعان و کنترل عفونت‌های بیمارستانی بسیار مهم است. پژوهش حاضر با هدف پایش شاخص‌های کیفی هوا شامل باکتری‌ها، ذرات معلق (PM10 و PM2.5)، ترکیبات آلی فرار (Volatile organic compounds یا VOCs)، مونوکسیدکربن (Carbon monoxide یا CO) و دی‌اکسیدکربن (Carbon dioxide یا CO2) در بخش‌های مراقبت ویژه (Intensive care unit یا ICU) بیمارستان‌های منتخب شهر اصفهان و بررسی ارتباط آن‌ها با عوامل محیطی انجام شد. روش‌ها: این مطالعه مقطعی- توصیفی در دو فصل پاییز سال 1401 و بهار سال 1402 انجام شد. در مجموع، از 72 مورد نمونه‌برداری در 18 ICU در 7 بیمارستان سنجش به عمل آمد. غلظت VOCs، CO2، CO، PM10 و PM2.5 با دستگاه‌های قرائت مستقیم اندازه‌گیری گردید. جهت نمونه‌برداری از باکتری‌های هوا، از ایمپینجر حاوی محلول جاذب بافر فسفات (Phosphate buffered saline یا PBS) و برای کشت از محیط تریپتیک سوی آگار (Tryptic Soy Agar یا TSA) حاوی نیستاتین استفاده شد. کلونی باکتری‌ها با روش رنگ‌آمیزی گرم و رؤیت در زیر میکروسکوپ مورد بررسی مورفولوژی قرار گرفت. عوامل محیطی مؤثر بر اساس استانداردهای کیفیت هوای داخلی ساختمان بررسی و ارتباط آن‌ها با شاخص‌های کیفی هوا از طریق آزمون ANOVA و همبستگی Pearson ارزیابی گردید. یافته‌ها: میانگین کل باکتری‌ها، باکتری‌های گرم مثبت و باکتری‌های گرم منفی به ترتیب 02/33، 52/31 و 52/1 واحد تشکیل کلونی در مترمکعب به دست آمد. میانگین غلظت PM10 و PM2.5 نیز به ترتیب 76/7 و 80/5 میکروگرم در مترمکعب و میانگین غلظت‌های VOCs، CO2 و CO به ترتیب ppm 08/0، 05/141 و غیر قابل تشخیص گزارش گردید. ارتباط معنی‌داری میان غلظت PM10، PM2.5، CO2 وCO و سطح باکتری‌ها مشاهده نشد، اما ارتباط معنی‌داری بین سطح سایر شاخص‌های کیفی هوا با عوامل محیطی مانند دما، رطوبت، تهویه هوا و درصد اشغال تخت‌ها وجود داشت. نتیجه‌گیری: اگرچه مقادیر بعضی شاخص‌های کیفی هوا در بعضی بیمارستان‌ها به ویژه در فصل پاییز بالاتر بود، اما از حدود استاندارد پیشنهادی World Health Organization (WHO) بیشتر نبود. مناسب بودن کیفیت هوا به خصوص از نظر شاخص‌های شیمیایی، نشان دهنده بهسازی بیمارستان‌ها، وجود سیستم تهویه، سیستم سرمایشی و گرمایشی مناسب، شستشوی مداوم ابزار و تجهیزات و حاکم بودن فشار مثبت هوا در بخش‌ها می‌باشد، اما برای ارتقای هرچه بیشتر کیفیت هوای ICU، رعایت استاندارد تعداد تخت‌ها، برقراری فشار مثبت هوا و محدودیت ملاقات توسط بستگان ضروری است.</div> <div style="text-align: justify;">Background: Improving air quality inside hospitals is very important for protecting the health of patients, staff, and visitors and controlling hospital infections. This study aimed to monitor indoor air quality parameters, including bacteria, particulate matter (PM10 and PM2.5), volatile organic compounds (VOCs), carbon monoxide (CO), and carbon dioxide (CO2) in the intensive care units (ICUs) of selected hospitals in Isfahan City, Iran, as well as investigating their relationship with environmental parameters. Methods: This descriptive cross-sectional study was conducted during two seasons, autumn 2022 and spring 2023. A total of 72 samples were collected and measured from 18 ICUs in 7 hospitals. The concentrations of VOCs, CO2, CO, PM10, and PM2.5 were measured with direct reading devices. An impinger containing phosphate buffer was used for sampling airborne bacteria, and tryptic soy agar (TSA) containing nystatin was used as culture media. Bacterial colonies were examined morphologically by Gram staining and observed under a microscope. Effective environmental factors were examined based on indoor air quality standards, and their relationship with air quality parameters was evaluated through statistical methods of analysis of variance (ANOVA) and Pearson correlation. Findings: The average of total bacteria, gram-positive bacteria, and gram-negative bacteria were 33.02, 31.52, and 1.52 CFU/m3, respectively. The average concentrations of PM10 and PM2.5 were 7.76 and 5.80 µg/m3, respectively, and the average concentrations of VOCs, CO2, and CO were determined to be 0.08, 141.05 ppm, and undetectable, respectively. Based on statistical analyses, no significant relationship was found between the concentrations of PM10, PM2.5, CO2, and CO and the level of bacteria, but the level of other air quality parameters was significantly related to some environmental factors such as temperature, humidity, air conditioning, and bed occupancy. Conclusion: Although the values of some air quality parameters were higher in some hospitals, especially in the autumn, they were generally not higher than the World Health Organization (WHO)-recommended standard limits. The suitability of air quality, especially in terms of the chemical parameters, indicates the improvement of hospitals, the presence of a ventilation system, an appropriate cooling and heating system, continuous washing of tools and equipment, and the prevalence of positive air pressure in the wards.
