fa فراوانی مصرف مواد، الکل، دخانیات و ارتباط آن با اندازه شبکه اجتماعی و حمایت اجتماعی در جمعیت عمومی 18 تا 49 ساله شهرستان لنجان، اصفهان Frequency of Drug, Alcohol, and Tobacco Use and Their Association with Social Network Size and Social Support among 18- to 49-Year-Olds in Lanjan, Isfahan, Iran آمار زیستی و اپیدمیولوژی Biostatistics and Epidemiology پژوهشي Research <div style="text-align: justify;">مقدمه: تأثیر مصرف مواد مخدر و روانگردان، الکل و دخانیات بر سلامت مردم قابل توجه است. این مصرف ممکن است تحت تأثیر عوامل مختلفی قرار داشته باشد. در پژوهش حاضر فراوانی مصرف مواد، الکل و دخانیات و ارتباط آن با اندازه شبکه اجتماعی و میزان حمایت اجتماعی در شهرستان لنجان مورد بررسی قرار گرفت. روش‌ها: 603 نفر از افراد 49-18 ساله ساکن شهرستان لنجان (زرین‌شهر و فولادشهر) از آبان سال 1401 تا اردیبهشت سال 1402 از طریق نمونه‌گیری چند مرحله‌ای خوشه‌ای در این مطالعه مقطعی شرکت نمودند. داده‌ها به وسیله پرسش‌نامه شبکه اجتماعی، نسخه فارسی پرسش‌نامه حمایت اجتماعی Norbeck (Norbeck Social Support Questionnaire یا NSSQ) و پرسش‌نامه رفتاری مصرف مواد والکل از طریق مصاحبه حضوری توسط پرسشگران مرد و زن در مراکز جامع خدمات سلامت جمع‌آوری گردید. از مدل رگرسیون لجستیک تک متغیره و چند گانه به منظور ارزیابی رابطه بین رفتارهای پرخطر و متغیرهای مورد بررسی استفاده شد. داده‌ها در نرم‌افزار Stata و سطح معنی‌داری 05/0 > P مورد تجزیه و تحلیل قرار گرفت. یافته‌ها: شیوع مصرف مواد، الکل و دخانیات در یک سال قبل از انجام تحقیق به ترتیب 93/14، 3/40 و 4/53 درصد بود. میانگین اندازه شبکه اجتماعی حدود 195 نفر و میانگین نمره حمایت اجتماعی، 134 به دست آمد. سن [030/0 = P، 97/0 = Odds ratio (OR)] رابطه معکوس و جنسیت مرد (001/0 > P، 01/8 = OR)، مطلقه بودن (010/0 = P، 72/3 = OR) و وضعیت محل سکونت استیجاری (070/0 = P، 78/1 = OR) رابطه مستقیمی با مصرف مواد، الکل و دخانیات داشت. بین مصرف مواد، الکل و دخانیات با اندازه شبکه اجتماعی و نمره حمایت اجتماعی ارتباط معنی‌داری مشاهده نشد (050/0 < P). مصرف مواد در افرادی که در طول عمر خود الکل و دخانیات مصرف کرده بودند، به ترتیب [32/16-36/4 = Confidence interval (CI) 95 درصد] 44/8 و (61/6-57/2 = CI 95 درصد) 91/4 برابر سایر افراد بود. نتیجه‌گیری: با توجه به قابل توجه بودن شیوع مصرف مواد، الکل و دخانیات در دو شهر فولادشهر و زرین‌شهر، پیشنهاد می‌شود مداخلات فوری جهت آموزش و افزایش آگاهی افراد پیرامون پیامدهای نامطلوب مصرف مواد به ویژه محرک‌ها، حشیش، نوشیدنی‌های الکلی و تغییر فرهنگ عادی‌سازی مصرف، برنامه‌ریزی و اجرا گردد.</div> <div style="text-align: justify;">Background: The effects of drugs, psychotropic substances, alcohol, and tobacco on health are significant. Their use may be influenced by various factors. This study investigated the frequency of drug, alcohol, and tobacco use and its relationship with the size of social networks and the level of social support in Lenjan County, Iran. Methods: This cross-sectional study included 603 individuals aged 18 to 49 residing in Lenjan County (Zarrinshahr and Foladshahr) from November 2022 to May 2023, selected through multistage cluster sampling. Data were collected via face-to-face interviews conducted by male and female interviewers at health service centers, using the Social Network Size, Nurnberg Social Support, and Walkol Substance Use Behavioral questionnaires. Univariate and multivariate logistic regression analyses were performed to examine the relationships between high-risk behaviors and the variables studied. Statistical significance was set at P < 0.05. Data analysis was conducted using Stata. Findings: The prevalence of drug, alcohol, and tobacco use in the year prior to the study was 14.93%, 40.3%, and 53.4%, respectively. The average size of the social network was approximately 195 people, and the average social support score was 134. Age was inversely related to substance use (P = 0.03; OR = 0.97), while male gender (P < 0.001; OR = 0.8), divorce (P = 0.01; OR = 3.72), and rental housing status (P = 0.07; OR = 1.78) were positively associated with drug, alcohol, and tobacco use. There was no significant relationship between substance use and social network size or social support score (P > 0.05). Substance use among individuals who have consumed alcohol and tobacco during their lifetime was 44.8 times (95% CI: 16.32-4.36) and 49.9 times (95% CI: 610.6-572.2) higher, respectively, than among others.
