Background: According to the high prevalence of diabetes in Iran, it is essential to achieve a reliable and valid instrument to evaluate the empowerment of diabetic patients in order to plan required interventions. Since approach to diabetes has changed from medical to empowerment approach, this study was planned to facilitate policymaking in health field. Methods: Validity and reliability of the Persian version of Diabetes Empowerment Scale-Long Form (DES-LF) were measured through a cross-sectional study. Content validity, reliability, and cultural equivalency of the Persian version of DES-LF were evaluated using qualitative (interviews and group discussions) and quantitative methods (mean, standard deviation, factor analysis, and Spearman's correlation) on 160 patients with type 2 diabetes who participated in the study. Findings: The reliability and validity of the scale and its 3 subscales, i.e. managing psychosocial aspects of diabetes (a = 0.94), assessing dissatisfaction and readiness to change (a = 0.96), and setting and achieving diabetes goals (a = 0.96), were approved by a psychometric analysis. A satisfactory intraclass correlation was found when a test- retest with a 2-week interval was conducted on 50 patients (P < 0.001). Criterion validity between global scale and metabolic control (hemoglobin A1c) of type 2 diabetic patients was approved (P < 0.001). Conclusion: Our findings approved the reliability and validity of the Persian version of DES-LF for patient education and psychosocial interventions among Iranian patients with type 2 diabetes.
