Background: Suicide and attempted suicide are mental complex phenomena which directly affected by personal and environmental factors. Since recognition of the risk factors for suicide and attempted suicide is very important, the aim of the present study was to recognize some of the most important risk factors of these two phenomena and their directions of function within Ilam province.  Methods: In the present study, registered data related to suicide and attempted suicide data by the systematic registry in Ilam province were studied. The registered data contained all suicide and attempted suicide cases and their demographic characteristics including age, sex, marital status and the method of suicide plus place and time of incident. Findings: Suicide in spring and methods such as hanging, firearm and self-immolation significantly increased and whilst to be married, being in age group 40-49 years and using methods such as poisoning and overdose significantly decreased the likelihood of committing suicide in comparison to committing attempted suicide (P < 0.05). Conclusion: Health and Medical centers within Ilam province in order to decrease the suicide rate should focus on primary prevention of those suicide cases that occur in singles, within spring and apply lethal methods such as hanging, firearm and self-immolation.
